1. Student's Birth Certificate

2. FOUR documents as proof of Chicago residency
(Utility Bills, Voter's Registration Card, Lease/Deed or Mortgage Statement, Driver's License, State ID)

3. Request for Records (if transferring from another school)

4. IEP/504 (if applicable)

5. CPS transcripts (if applicable).  

6. Medical/Vaccination Records
